APPLICATION FOR BRANCH CAMPUS
Branch Requirement: Make initial non-refundable $200.00 Affiliation Fee Payment to this application. Complete all portions of the application correctly for submission. Application will be reviewed for verification of non-profit, faith based organization and church.
Each organization is required to have a legal physical address for operation. Member branches are expected to pay an annual $100.00 renewal fee to retain membership annually. Upon approval, each branch will receive funding partnership agreement for operational expense.
